A new program by  Greater Sudbury Paramedic Services will look to cut down on hospital readmissions and help seniors and vulnerable populations enjoy life at home longer.

The Community Paramedicine Program is broken down into two ten month pilot projects, the Transitions Care Program, and the Health Promotions Program.

Director Of Nursing Programs at the North East Community Care Access Centre, Martha Musicco says a major part of Health Promotions is understanding.

Musicco says these programs are key with our aging population, and the Transition Care Program will focus on keeping patients in their home longer, while Health Promotions will be focusing on education.

Three paramedics have been assigned to operate the two projects.

The Paramedic Services are partnering with North East CCAC, and Health Sciences North, for the programs that are fully funded by the Ministry of Health and Long-Term Care.
